To Schedule An AppointmentCall (870) 932-1198 or Toll Free (888) 798-0700 to schedule an appointment Monday - Friday 8:30am - 4:30pm. ORLogin to your Patient Portal account and complete the "Request an Appointment" form under the "Send Medical Info" menu. Your physician's nurse can then call you back to confirm date and time of appointment. Available anytime 24 hours a day, 7 days a week. Requests submitted will be processed by our clinic staff as soon as possible. You will hear from us within one business day. This should not be used in the case of an emergency. If you are having an emergency, please call the clinic or emergency personnel (911) directly.
